1. The 2019 ACR/Arthritis Foundation guideline makes STRONG recommendations for exercise, weight loss in people with overweight or obesity, self-efficacy and self-management programmes, tai chi, cane use and oral NSAIDs across hand, hip and knee OA - but its strong recommendations for topical NSAIDs and for intra-articular glucocorticoid injection are specific to the KNEE, not the hip.

2. A Bayesian network meta-analysis of 11 randomized trials (1353 patients) in hip osteoarthritis found that at 2-4 months and 6 months NO injectable - corticosteroid, hyaluronic acid or platelet-rich plasma - significantly outperformed a saline placebo injection for pain or function. Pooled change from baseline exceeded the minimal clinically important difference in every arm including placebo.

3. The AAOS evidence-based clinical practice guideline on Management of Osteoarthritis of the Hip states verbatim: STRONG evidence supports intra-articular corticosteroids to improve function and reduce pain in the SHORT TERM; STRONG evidence does NOT support intra-articular hyaluronic acid, because it does not perform better than placebo for function, stiffness and pain; STRONG evidence supports physical therapy for mild to moderate symptoms; STRONG evidence supports NSAIDs for short-term pain and function; and MODERATE evidence does not support glucosamine sulfate.

4. Pooled analysis of national joint registries (Australia and Finland, 215,676 hip replacements) put 25-year all-cause construct survival at 57.9% (95% CI 57.1-58.7); pooled case series gave 77.6% (95% CI 76.0-79.2). The authors conclude patients and surgeons can expect a hip replacement to last 25 years in around 58% of patients.

FDA states verbatim that regenerative medicine products including stem cells, stromal vascular fraction, umbilical cord blood, Wharton's jelly, amniotic fluid and exosomes have NOT been approved 'for the treatment of any orthopedic condition, such as osteoarthritis, tendonitis, disc disease, tennis elbow, back pain, hip pain, knee pain, neck pain, or shoulder pain.' The only FDA-approved stem cell products in the United States are blood-forming (hematopoietic progenitor) cells derived from umbilical cord blood, approved only for disorders affecting blood production.

## What usually causes hip soreness?

Deep groin soreness often comes from wear inside the ball-and-socket joint. Pain over the outside bump often begins in a tendon, the cord joining muscle to bone.

Burning or tingling below the knee may come from the back or nerves. An exam helps separate these causes.

## Which hip warning signs need quick care?

Get urgent care for a hot, swollen joint with fever or severe sudden soreness. A fall followed by trouble bearing weight also needs prompt help.

New weakness, spreading numbness, or loss of bladder or bowel control also needs emergency care. Don't wait for an office appointment with these signs.

## What can I try at home first?

Keep the hip moving gently, because full rest can increase stiffness. Try shorter walks and change one load, such as distance or stairs, at a time.

Heat may loosen stiffness, while cold may settle soreness after activity. Ask your doctor whether pain medicine is safe with your health history.

## What does PRP mean?

PRP means platelet-rich plasma. It is prepared by separating a sample of your blood and keeping the platelet-rich portion for clinic use.

Results for hip soreness are mixed, and the preparation can differ. Ask what is being used, what the exam found, and what result is realistic.

## Can I avoid hip replacement?

Exercise, pacing, a cane, and suitable medicine may ease symptoms or help you move. They can't rebuild a badly worn hip.

Surgery becomes worth discussing when rest soreness, poor sleep, and lost independence remain severe. Getting an opinion gives you facts; it doesn't commit you to an operation.
